UPNEEQ (oxymetazoline hydrochloride ophthalmic solution, 0.1%) is the first and only FDA-approved prescription eye drop designed to treat acquired blepharoptosis, the clinical term for low-lying or drooping upper eyelids. The Science and Safety of UPNEEQ

How It Works

UPNEEQ contains oxymetazoline hydrochloride, an alpha adrenergic agonist that acts on the Muller's muscle in the upper eyelid. When one drop is applied, the active ingredient stimulates this muscle to contract, which raises the upper eyelid and opens the eye. This is the same class of active ingredient found in decongestant nasal sprays, but here it is formulated specifically for ophthalmic use to target eyelid elevation rather than nasal vessels.


Each single-use vial contains enough solution to treat one or both eyes. Results have been shown to last at least 6 hours per dose in clinical studies, and some patients experience effects for up to 8 to 10 hours.

Clinical Evidence

FDA approval for UPNEEQ was supported by two 6-week clinical trials involving 304 adults with acquired blepharoptosis, plus a third study confirming safety. Results showed that 84% of patients experienced improvement in eyelid lift, and 74% had more than 50% improvement. The average upper eyelid lift across studies was approximately 1 mm, which is clinically meaningful for both appearance and field of vision.


The most commonly observed side effects, reported in 1 to 5% of study participants, included mild eye inflammation, redness, dry eye, blurred vision, eye irritation, and headache. UPNEEQ should not be used by patients with certain underlying conditions including cardiovascular issues, uncontrolled hypertension, or specific neurological conditions.
